What happens when inaccurate data about a person are processed?

The public body processing personal data has an obligation to make certain it is accurate. In the event that there is inaccuracy in the data, the data subject is always entitled to have their data corrected. But how is the decision made on what is accurate and what is not? And when is it possible to determine the accuracy of data, and when not?
